US
Holon Health pairs a bio-psych-social care team with a proprietary digital therapeutic to help people with substance use disorder (SUD) fill gaps in care and remove social obstacles that often limit their ability to engage in treatment and negatively affect adherence. Our digital therapeutic application augments mental health and SUD treatment while promoting healthy activities and lifestyle. This combination of people and technology simplifies complexities, positions people with substance use disorder to achieve improved health, and aims to decrease healthcare expense through fewer emergency room visits and inappropriate utilizations of high-cost services, to reduce hospital (re)/admissions, and to focus on preventive health care for chronic health conditions related to substance use disorder.

ABOUT TRACI SWEET
As the Co-founder and Chief Clinical Officer of award-winning Holon Health, Inc. and a nationally recognized subject matter expert in behavioral health and substance use disorders, I’ve spent over 30 years transforming how care is delivered, especially for those with high-acuity, those mislabeled as \"Treatment Resistant,\" and the underserved, complex populations. My mission is to challenge the status quo of outdated models and prove, with data and outcomes, that there is always a better way.I specialize in developing scalable, evidence-based programs, digital therapeutics, and user-friendly clinical protocols that sustain change, reduce relapse, close treatment gaps, and improve the patient experience. From the frontlines of care to national strategy tables, I bridge clinical rigor with innovation by helping providers, payers, and systems rethink what recovery and resilience can truly look like.I’ve authored peer-reviewed articles, spoken on leading national podcasts, and trained thousands of professionals across the country. My work spans provider and payer relations, value-based contracting, curriculum development, and systems-level transformation.I’m also the author of the forthcoming book,\"Try This Instead: Why Willpower Fails and What Works Instead,\" a modern, evidence-based guide to personal change for people who feel like they’ve tried everything. I introduce my proprietary model of change and dispel the myths of willpower and motivation that have led people to feel shame and failure by refocusing attention on what I have observed leads to sustained change in thousands of clients I have worked with in my career to date.Personally, having overcome my own health challenges, I became a functional medicine practitioner and a health optimization and peptide science geek committed to sharing my knowledge and experience in this area to help others navigate and beat their own health challenges and optimize their longevity.Core Strengths:Innovation & Strategy in Evidence-Based TreatmentPublic Speaking, Training, and National Thought LeadershipClinical Program Design & Scalable InterventionsTechnology in Behavioral Health & Digital TherapeuticsStrategic Planning, M&A, and Value-Based CareFollow me and let’s connect if you’re working on bold solutions that move behavioral healthcare forward.
