Signify Health is the leading provider of Health Risk Evaluations (HRE\'s) for healthcare insurance plans and healthcare delivery organizations. Since 2009, clinicians in their network have completed more than 3.5 million health evaluations for major insurance providers like BlueCross-BlueShield, Humana, Aetna, UnitedHealth, etc. Each year, Signify Health completes more than 1 million evaluations for members across the country. As an evaluating physician and supported by technology focused streamline interactions, I perform in‐home and telehealth evaluations and assessments of members’ health, medical histories, medications and perform full physical exams to drive better clinical outcomes through care management referrals and return‐to‐care recommendations. This in turn improves member health engagement, improve quality ratings and reduce overall healthcare costs.
