During my tenure at VTA in the Bus and Rail section as a System Safety Officer, I was responsible for various tasks, including:Risk Assessment and Analysis: My role involved reviewing crossing hazards for pedestrians and vehicles, drafting corrective actions and supports aimed at enhancing the safety of the bus and rail system, particularly in areas such as crossings, equipment, machinery, and vehicles.Incident Investigation: I conducted thorough investigations into transportation system incidents with the goal of identifying potential safety hazards and taking steps to prevent future accidents.Transit Incident Reporting to Regulatory Agencies such as FTA, FRA & CPUC: I played my role in conducting deep investigations for state and federal transit authorities. This ensured that our reporting was accurate, timely, and in compliance with regulatory requirements.Field Safety Observation and Analysis: I actively engaged in on-site safety observations within the field. This involved assessing potential risks and meticulously analyzing safety procedures to identify areas where improvements could be made.Near Miss data Analysis: This report provides a comprehensive analysis of quarterly and annual near miss occurrences involving bicyclists, vehicles, and pedestrians. The goal is to identify patterns, assess risks, and recommend safety improvements. The analysis covers the latest data available to ensure informed decision-making by safety committees.Safety enhancement suggestion for agency-wide risks regarding the LRV equipment brake, Bell and horn sound and directions, Track defects and priorities, Non-gatted crossing safety and visibilities, and MUTCD safety at in-grade crossing.
