Oncology: Verify chemotherapy and supportive regimen by comparing with NCCN guidelines to optimize therapy plan, prevent/reduce medication error, and establish proper monitoring plans. Transition of Care: Perform medication reconciliation by tactfully accessing and communicating different sources; identify any discrepancies and implement evidence-based recommendation by collaborating with other healthcare providers. Sterile Preparation: Prepare, supervise, and ensure aseptic compounding of IVs including antineoplastic agents correctly and in a timely manner promoting efficiency and cost effectiveness. Review all patient charts for correct medication review and documentation, including patient drug allergies, drug/drug interactions with current medications, appropriate medication choice, route of administration and dose for therapy optimization. Clinical duties include renal dosing, drug regimen review, antibiotic & anticoagulant monitoring. Consults with and advises Administration, Medical Staff and hospital staff on quality, regulatory and risk issues within the organization.
