Collaborate with the Clinical Care Management team, helping patients cope with the stress of hospitalization, and connecting them with appropriate community resources. Core Responsibilities:Psychosocial Assessments:Conducting thorough assessments to understand patients\' emotional, social, and psychological needs related to their illness and hospitalization.Crisis Intervention:Providing immediate support and guidance to patients and families facing emotional distress or experiencing a crisis due to the patient\'s condition or situation.Discharge Planning:Working with the care management team to develop and implement discharge plans that address the patient\'s needs for ongoing care, including referrals to community resources.Education and Support:Educating patients and their families about available resources, treatment options, and coping strategies.Collaboration:Partnering with the multidisciplinary care team, including physicians, nurses, and other healthcare professionals, to ensure comprehensive patient care.Documentation:Maintaining accurate and timely records of assessments, interventions, and discharge plans. Specific Tasks:Identifying and reporting abuse/neglect:Serving as a mandated reporter and collaborating with social services as needed. Facilitating communication:Acting as a liaison between the hospital, patients, and their families. Advocating for patients\' rights:Ensuring patients\' needs and preferences are respected throughout their ED visit. Addressing social determinants of health:Identifying and addressing factors like poverty, lack of transportation, or housing instability that may impact a patient\'s health.

I am also the founder of Tranquil Talk with Kenya, a private practice where I provide personalized, unorthodox talk therapy for those who may have been skeptical about therapy or struggled to find the right fit.Earlier in my career, I gained valuable experience as a nonprofit leader serving homeless families with minor children, and later as a Community Service Manager overseeing local government programs.
