PROJECT 1 – TITLE: ESTABLISHING COMMUNITY-BASED PRIMARY PALLIATIVE CARE IN THE LOW RESOURCED URBAN AREAS SERVED BY LOW-COST EFFECTIVE CARE UNIT (LCECU), CMC VELLORE, 20••••26- Identify people with one or more chronic illnesses from the low-resource urban population of Vellore town served by LCECU using the SPICT-LIS tool for integration of palliative care in chronic disease management- Enumerate the symptom burden among the identified population using the PACA tool (Palliative Care Assessment Tool)- Plan holistic management of patients identified for integration of palliative care- Integrate care with palliative care specialist team for patients with complex needs- Sensitize and increase awareness on benefits of palliative care within the community and thus improve community support- Coordinating the day-to-day activities of the program and data management- Networking between the community, LCECU, and Palliative medicine unit- Record keeping- Report generation and submission- SM & E.PROJECT 2 – TITLE: IMPLEMENTING COMMUNITY-BASED CERVICAL CANCER SCREENING IN THE LOW-RESOURCED URBAN AREAS SERVED BY LOW-COST EFFECTIVE CARE UNIT, CMC VELLORE- Create awareness regarding cervical cancer, screening for cervical cancer, treatment of pre-invasive cancers and HPV vaccine in the community- Motivate and counsel the eligible women to participate in the screening- Screen women in the age group 30-60 years using Visual Inspection with Acetic acid (VIA) to identify pre-invasive cervical lesions at the community level- Coordinate treatment of those with abnormal lesions at the community level by colposcopy and thermo-coagulation- Provide referral services to the tertiary care center (CMCH) when needed- Coordinating the day-to-day activities of the program, data management and record keeping- SM & E.
