Conduct daily individual ward rounds to determine and optimize therapies provided to the patients admitted under Medicine, Oncology, Cardiology, Neurology, Infectious Diseases, Nephrology, Hematology and Oncology department. • Assist in medical decision making for choice of antimicrobials as part of Antimicrobial Stewardship Program.• Ensure accurate and safe administration of High Alert Medications.• Perform admission and discharge medication reconciliation, and recommending changes. • Monitor compliance of antimicrobials in surgical prophylaxis as per the policy.• Assess renal and hepatic dose adjustments on patient – specific conditions.• Evaluate and rectify the days of therapy for reserved antimicrobials.• Prepare and deliver CME and CNE\'s to healthcare professionals.• Identify Medication Errors, perform their RCA and implement CAPA• Identify and report Adverse Drug Reactions to PVPI. • Attend PTC, Causality Assessment, Quality, and Hospital Infection Control meetings and providing relevant inputs. • Ensure quality compliance of Good Prescription Practices (GPP), and Management of Medication (MOM) as per NABH guidelines. • Review literature to gain updates about recent advancements with respect to diseases and therapies.
