Inpatient Job Duties- Prepare/reconcile inpatient/outpatient Chemos, Ped/Adult TPNs, Operate Narcotic pyxis, check all code trays and kits. Perform Clinical duties: IV to PO/Renal adjustment, ABX Streamlining, IV/TPN Counseling, Transition of care on admission/discharge of patients, and initiate monitoring/education to new patients on Coumadin, Xeralto, CHF Patients. Perform Central Order Verification (COV) and page MD for order clarification with documentation of clinical interventions. Review pertinent lab values prior to processing med orders. Centralized in IV room to check and verify all compounded IVs, TPNs (operate Compounder System), and Chemo to ensure usage of appropriate IV solution, bag (DEHP vs Non-DEHP), tubing and filter. Charge Pharmacist duties to check and verify all medications and ensure med delivery to inpatients on floors. Reconcile IV/Oral /Cart-fill Batch sheets and labels. Med Reconciliation (Admission), liaise with Hospital Floor Nurses/Doctors to monitor meds for inpatients, and implement all other clinical initiatives. Outpatients Job Duties- Provide efficient pharmacy operations and processes to ensure excellence in patient care and safety. Dispense and distribute medications to patients and advise on correct dosage, proper usage and compliance. Advise physicians/practitioners on inherent dangers of medications and eventual side effects and assist in prescribing the correct doses. Work with other health care professionals to plan, monitor, review and evaluate patient effectiveness, monitor patients for side effects and drug interactions.
