Manage medically fragile children\'s care and services under the Consolidated Children\'s Waiver. Mandated monthly meetings with caregivers for updates on child\'s condition and services receiving or needed. Monthly reports due for health home required. Coordinate with other Care Managers on procedures, contacts, guidelines, etc. Work closely with County and State Department of Social Services for needs of children. Research organizations that provide help for equipment and services not covered by primary insurance and/or Medicaid.*Assess families on the CANS and collaborate with them to develop a Comprehensive Care Plan and note in EMR.* Provide services identified in the Comprehensive Case management Plan; evaluate outcomes and reassess as needed.*Collaborate with various agency\'s staff, community based providers, interdisciplinary teams for management of care plan.*Participate in Continuing Education; program mandates/changes; community resources and access information; professional development and CMA competencies mandated by the lead Home Health Agency.*Maintain case notes documenting mandated case activity, plans and outcomes for each family in accordance with established documentation guidelines created by the Health Home Model.*Comply with established Company policies and procedures re: quality assurance, safety, environmental and infection control, PHI, HIPPA, customer service, professional conduct, annual health assessments, etc.*Attend supervisory meetings as requested, at least monthly.

I have a Bachelor\'s Degree in Health Sciences-Public Health. My studies provided knowledge surrounding health policies and services, epidemiology, global health, experimental studies and clinical trials. I have acquired research, project, prevention and education skills. I am currently working as a care manager for medically fragile, disabled children. Client advocacy is a crucial; always reaching out to get them services needed.I am dedicated to lifelong learning, consistently working toward expanding my knowledge of new medical practices and health research. I have been involved in research studies for a past client. This is where I developed my passion for clinical research.I had been an accomplished and compassionate LPN with seventeen years of working with a variety of patient populations. I have extensive experience with medically fragile pediatrics, chronic disease, while always being an effective patient and family educator. During my 15 year experience as a New York City Police Detective, I developed a passion for field investigations. Through hands on experience, it became second nature working with a wide variety of people, cultures and situations.Through my studies of Public Health and working across several nursing disciplines, I acquired a strong interest of population studies and research surrounding the spread of infectious and chronic diseases.With much enthusiasm for my future, I want to pursue a position within a challenging environment.
