My Vision - A World Where Every Person Living with Chronic Disease Is Empowered To Live A Life Of Health, Wellness, And Hope Without Guilt, Blame, And Shame.My Mission- Increase Access to Outcome Driven, Member Experience Focused Solutions & Support for All.I have used my diagnosis of diabetes as the catalyst to pursue my mission to bring access to outcomes-driven education programs for all people living with diabetes.Tony Song is the CEO and co-founder of Diabetes Care Partners. A direct patient care provider organization of evidence-based diabetes self-management training and CDC diabetes prevention programs utilizing telehealth to maximize access in underserved populations. With over two decades of experience, he serves as an expert consultant for many organizations wishing to optimize diabetes program enrollment, engagement, utilization, and outcomes to achieve higher patient satisfaction and improved HEDIS and CMS Star measures. As a pioneer in the diabetes industry, He speaks locally and nationally on the topic of innovative systems of population management, patient-centered curriculum development, and telehealth implementation and sustainability for organizations such as the CDC, the National Association of Chronic Care Directors, The Los Angeles Public Health Department and the Association of Diabetes Care and Education Specialist. As a community leader, he has served on boards of directors for various healthcare, childhood education, and philanthropic foundations and is currently Co-Chair of the Los Angeles County Department of Public Health Diabetes Coalition.He is a proud graduate of the University of Southern California with a Bachelor of Science in Molecular Biology andHealthcare MBA at the University of Northern Arizona W.A. Frank College of Business.
