As a Practice Transformation Specialist (PTS) at Aledade, I have spent the past three years personally making an impact on patient care by supporting primary care practices in their transformation to value-based care. In my role, I am entrusted with guiding physicians and staff through the transition to new workflows, integrating technology, and driving key initiatives aimed at improving patient outcomes and reducing healthcare costs.Key Responsibilities:Project Management: Lead and manage projects related to practice transformation, ensuring that workflows, clinical calls, and workgroup participation are aligned with practice goals.Data Reporting & Analysis: Export data, run reports, and track key performance metrics to assess progress toward value-based care outcomes.Problem Solving & Improvement Plans: Identify and address obstacles in workflows and processes, proposing solutions to streamline operations and enhance care delivery.Collaboration & Communication: Partner with Medical Directors and Executive Directors to ensure timely project execution, clear communication of goals, and alignment on strategies for achieving success.Staff Training & Technology Integration: Design and implement training programs for practice staff, ensuring the seamless incorporation of Aledade\'s technology and processes into daily practice operations.Leadership & Best Practices Sharing: Mentor and collaborate with teammates across Aledade, sharing successful strategies and lessons learned to improve care across the network.Recent Key Achievement:Oversight of Medicare Advantage Committee (California Market): I have recently taken on the leadership of the Medicare Advantage Committee, currently overseeing 15 primary care practices in California as they work toward shared goals under Accountable Care Organizations (ACOs). This initiative is a critical part of our mission to improve value-based care and ensure better patient outcomes while reducing unnecessary costs.
