   As a Marketing and Communications Specialist my responsibilities are to plan and coordinate effective programs for the delivery of professional presentations and community education with regards to health, home health care and affiliated services. Additionally, I am challenged with establishing and maintaining lines of communication to promote home care objectives with key constituency groups: physicians, employees, patients, extended care facilities and the community.Responsibilities:• Assist in development and preparation of the home care marketing budget.• Act as liaison between referral sources and home health agency. • Coordinate community education, health outreach programs, and professional relations activities for the home health services division. • Develop contacts and provide educational information to physicians in the community to outline services offered through the home health division and how it meets patient needs throughout the care continuum.• Design, prepare and disseminates educational materials for the community and professional groups and committees.• Represent home health services divisions at community awareness activities, hospital functions, public health and nursing association meetings, medical society gatherings and similar groups.• Present regular reports of community education activities to the home health services division.• Prepare monthly reports detailing “Key Result Areas” as it relates to marketing and financial growth.
