   As a Medical Social Worker, I\'m responsible to provide social work services for an assigned patient caseload, to facilitate patient admissions and/or initial psychosocial assessments, and/or to make visits to meet Medicare guidelines and patient and families needs.Additionally, I\'m responsible for completing psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation. Based on the assessment, I develop, and implement a comprehensive psychosocial hospice plan of care utilizing appropriate clinical social work treatments and intervention, while maintaining the dignity of the dying patient. I\'m responsible for providing education to patients and families around issues related to adaptation to the patient\'s diagnosis, illness, treatment and/or life situation.Furthermore, I participate and collaborate in the Interdisciplinary care team meetings to provide the social work perspective on the patient and family. I also collaborate with facility staff members to determine the patient’s level of care and coordinate the appropriate utilization of spiritual/cultural beliefs by assessing for cultural issues; developing culturally sensitive care plans based on patient/family wishes.
