This involves:*Providing personalized guidance to help them make sustainable lifestyle changes, including nutrition, physical activity, and mindset shifts.*Collaborating with multidisciplinary teams, including dietitians and healthcare professionals, to deliver evidence-based interventions.*Supporting clients in navigating their challenges, celebrating their successes, and staying motivated throughout the program using BCT and MI.*Utilizing digital tools and resources to enhance client engagement and track progress effectively.Working within this NHS-supported initiative allows me to make a meaningful impact by helping people take control of their health and reduce their dependence on medications—all while contributing to groundbreaking efforts in diabetes management.
