Responsible & work with the NCD team at state and coordinate with allied and districts NCD unit in implementing NCD and CPHC program.• Working with NCD Districts unit is facilitating implementation of population based screening for diabetes, hypertension and cancers.• Conduct state and sub-district/ CHC level trainings for capacity building on NCD and CPHC.• Assist administrative and logistical support to the project lead and liaise with different stakeholders of programme.• Maintain database, maintain project\'s monitoring and evaluation records and provide process consultant expense report.• Maintain and update state and district database of NCD diseases including cancer, diabetes, cardiovascular disease and stroke.• Provide mentoring and supportive supervision to field level functionaries.• Maintaining district level data on physical, financial, epidemiological progress.• Effective convergence with NHM activities.• Strong in stakeholder management.
