Coordination of services including diversion, homelessness prevention, linking households to qualifying referral sources, sending appropriate referrals to Continuum of Care service providers, on-going case management, and case conferencing. Daily data entry into the Homeless Management Information System (HMIS) and CAP 60Identify new opportunities for growth with in the county to achieve a decline in homeless population by placing them in safe environments Collaborate with local government and other agencies to improve assistance given to our vulnerable population
