About David Pharmacist
I am an Ohio (and KY, MS, AR, VA, NV, GA) licensed pharmacist seeking to expand my career focusing on great patient care & teamwork. I have experience in retail, hospital, and managed care pharmacy, and I am a 10 year+ member of the American Society of Consultant Pharmacists. I most enjoy setting up new programs and analyzing unclear information. Where there is an unclear path to success, I have implemented solutions that have been successful and impactful to patients on a corporate-wide scale. Proactively, or in response to the needs of a team, I collect information that reveals a way to address the root-cause of issues. As of August 2025, I have been optimizing how my team coaches to improve motivational interviewing among P3 students.

I verify that data entry has been completed accurately, and evaluate the clinical appropriateness, legality, and audit risk of orders. 20••••16 As a member of the Medication Safety Committee we discussed recent misfills and near misses and developed plans to avoid those issues in the future. Publications included: thyroid product conversion, long-term dosing of azithromycin, similar prednisolone products, and niacinamide\'s place in therapy, griseofulvin’s interaction with birth control & risk factors for QT prolongation.20••••18 As a member and the secretary of the Therapeutics and Standards Committee I helped to develop new standards to increase consistency and timeliness of centrally processed prescriptions. Projects included: determining policy on abbreviations, determining policy on written quantities that are a fraction of a box size (I created a guidance document on this), determining how to treat vaguely written prescriptions.Fall of 2016- Present I began completing CMRs, over the phone, through the OutcomesMTM platform. This currently accounts for 0.2-0.3 FTE. About half of this time is spent in preparing CMRs for pharmacists to deliver face-to-face.In March of 2017 I began co-leading the Medication Safety Committee for a 1 year assignment-it accounts for about 0.2 FTE. I am responsible for improving the accuracy of 100 pharmacists at 2 locations—over the course of the fiscal year we improved overall error rate by 20%. This is accomplished with a team of 10 in Cincinnati and a team of 5 in Columbus—I share many responsibilities with the leader in Columbus. I was responsible for leading the development and communication of: 1) weekly e-mail alerts, 2) monthly huddles & quizzes, 3) and quarterly medication safety PowerPoints & newsletters.
