   Responsible for operationalizing all value-based initiatives, which includes creating standard operating procedures, forms, and training materials. Collaborates with members of the Clinical Innovation Department to maximize clinicians’ ability to capture applicable Hierarchical Condition Categories (HCC) at the point of care. Programmatically reviews clinicians’ performance in capturing HCC conditions and formulating an associated plan of care. Recommendations are given in conjunction with the Chief Medical Officer (CMO) and Director of Clinical Education. Assists the Human Resources Department in training frontline staff on the principles of value-based care. Establishes and monitors the conduction of point of care testing based on clinical criteria and for the purpose of prevention, early detection, and HCC captures. Such examinations include screenings for peripheral artery disease and neuropathy and spirometry. Coordinates with office managers and clinicians on plan of care adherence for patients representing high risk. Such compliance includes participation in remote patient monitoring (RPM), chronic care management (CCM), specialty care, and monthly subsequent visits with the primary care physician (PCP). On a monthly basis, reviews the organizational performance with the Chief Value Officer (CVO) on current medical loss ratios (MLR) and risk adjusted factor (RAF) scores for all contracted third-party payers and enrollments to the Direct Contracting Entity (DCE). Recommendations of improvement are provided to other members of the executive team. Manages a team of care coordinators to navigate value-based and high-risk patients to relinquish barriers to care. Creates operational processes that link medically necessary services from community partners with vulnerable patients such as homebound modalities.
