Senior Manager Population Health @Kaiser Permanente

Senior Manager provides oversight, accountability and responsibility for population health, care management and panel management for specific populations. This oversight includes quality improvement, addressing healthcare disparities, care gap closures and safety net functions within the KPGA market. The Senior Manager is responsible for the development, implementation, supervision and evaluation of new and existing population health programs aimed to positively impact members ability to effectively self-manage their chronic condition(s) and achieve optimal health and wellness. In alignment with this responsibility, the Senior Manager, Population Health Management will oversee the regional clinical quality strategy, annual clinical quality objectives and regulatory requirements. They will proactively track metrics (both clinical and non-clinical), staff productivity and efficiency, as well as implement strategies to support HEDIS improvement and Medicare Stars performance in alignment with the Complete Care model of care. The Senior Manager will work in collaboration with Quality and Operational leaders as well as other KFHP and TSPMG leaders to ensure KPGA members receive the highest quality care.\\\\n \\\\n\\\\n \\\\n \\\\n\\\\n \\\\n
