As a seasoned corporate health systems executive leader and a family physician, I have experience in urban and rural healthcare environments in the US and around the globe. Governance and organizational structure:Organizational structure and governance gives us the platform that allows healthcare professionals to come together to collectively to address the issues facing healthcare today. A platform where stakeholders have a voice in influencing decisions creates an environment of ownership and accountability. I have had the opportunity to design and implement physician compensation models that reward individual behaviors to deliver care at the highest levels and service standards, while meeting over-arching organizational needs.Size matters: Healthcare systems need to be large enough to take risks and provide needed resources.During my tenure as President of Sanford Clinic, we grew the organization from fewer than 200 physicians to 1,500 employed physicians located across 5 states with an annual operational budget of $1.5 billion. The operation included ambulatory primary care, specialty clinics, and ancillary services. The organization’s culture should reward a multi-disciplinary, cross-functional team approach to address the patient’s needs at the highest levels of service and quality standards, allocating resources for the right care, to the right patient, at the right time. Innovative solutions for addressing barriers to care access and social drivers of both health and illness should be encouraged. Visionary, strategic, and transformational change: Efforts to FORCE change are doomed from the beginning. Change that is guided through strong trust relationships, a collaborative approach and engagement of all stakeholders is much more likely to yield transformative change.
