This approach readily identifies changes in patient behavior and care staff are more likely to notice slight changes in a patient\'s health which, in turn, can prevent escalation of more serious health problems. Every patient is assessed by a Registered Nurse prior to the Start of Care. We then create a customized Plan of Care (POC) specific to the patient\'s needs.
