Clinical Supervisor (CS) is responsible for managing the provision of all therapeutic care and services provided to patients within the home. CS oversees all aspects of coordination of care for patients from admission through discharge of service. CS is also accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating patient needs, including communication with the patient\'s interdisciplinary teams/specialists to address emergent or ongoing concerns related to diagnosis, medically necessary devices, or acute changes in patient condition. Clinical supervisor acts as the advocate and resource for patients and field clinical team members to ensure appropriate, evidenced based care is provided efficiently and effectively within the home care setting. Field team members are trained and supported by RN Clinical Supervisors to ensure field members are up-to-date on evidenced based practices and are able to support, educate, and reinforce patient specific goals and medical needs to caregivers and patients when in the home.
