Start a De Novo (new satellite office) by doing community events in the area, building relationships, and trust along with education on Altus and how we stand apart from other hospice companies in a new communityJoin the Chamber of Commerce San Marcos and New Braunfels to participate in Community events and grow the Altus name in a new territoryDevelop relationships with Hospitals, Skilled nursing facilities, Assisted living and Memory Care to be able to serve our local community and help with Hospice needsCold call, Grow, and Maintain Book of Business of approximately 100 contacts in the San Marcos, New Braunfels, Luling, Canyon Lake, and Wimberley communityRepresent Altus in the community by providing In-service and Education on Hospice Services Build, Grow, and Maintain relationships with all Referral sources, Physicians, Case Managers, Social Workers, Nurse Practitioners, Hospitals, and Nursing FacilitiesInitiate and coordinate contract negotiations with Nursing Facilities, Insurance Companies, and Managed care organizations Discover the patient\'s greatest concern, have goals of care conversationProvide education to patient and family on end of life care, help with placement if needed by working with the facilities in the areaMeet with patient/family to sign hospice paperwork/notaryResearch and implement placement for a patient when neededContinuous communication with the referral source during discharge process Organize durable medical equipment needs for patient and have delivered to the home before discharge Schedule transportation via ambulance for patient to discharge home on hospice Follow up with referral source after patient is admitted to hospice and update throughout the patient’s process to end of life
