As one of 58 Ontario Health Teams across the province, we are dedicated to strengthening an interconnected health care system. Alongside our 90 members of diverse health care and social service organizations and providers, we’re focused on building a local infrastructure that enables us to come together and deliver equitable, timely access to health care by integrating expertise and perspectives across the health care continuum. Our goal is to integrate health care plans, support providers and innovate to address disparities, all while simplifying access and improving the overall health of our communities. Together, we’re making Mississauga a healthier, more connected place to live.As part of the Community Health Advisory Network CHAN) I provide guidance on creating an inclusive care system that is easy to navigate and effective for everyone. CHAN consists of 11 members, including representatives from equity-deserving communities such as Indigenous, Francophone and Black populations. The group meets monthly to provide feedback on Mississauga Health initiatives in addition to developing and supporting the implementation of a community engagement plan. The Community Health Advisory Network (CHAN) is the driving force behind Mississauga Health’s mission to build an equitable, person-centered health care system. We are a diverse collective of patients and caregivers, providing essential insights and firsthand experiences that shape the health care and social services and programs offered in our region. As part of my responsibilities, I also sit on the Older Adult Healthcare and Digital Health Transformation Planning Tables when I provide my lived experience, supporting my 98 year old mother, and my professional experience leading automation and digital businesses.
