   Our tech-enabled solutions-as-a-service offerings provide access and then guide patients through the challenges of treatment and recovery — providing knowledgeable care guides to improve patient engagement, satisfaction and efficient resource utilization by interacting with patients in between the patients’ visits to clinical settings — the exact gaps when and where obstacles are most likely to occur. Our technology-enabled service fuses specially trained, non-clinical workers with a sophisticated tech platform (HL-7 listening engine, rules engine, predictive analytics, structured workflows, an extensive library of non-clinical interventions, and a communications platform (voice, SMS, email – all automatically triggered or independently initiated, inbound or outbound) to identify – and help patients resolve - barriers, obstacles and issues that are embedded in the Social Determinants of Health and which impede patient compliance with clinical instructions. Our care guides engage patients and intervene to address issues in ways that help the patients better manage their care journey. Everything we do is disease- or condition-specific. For instance, the kinds of engagement we have, and the topics we cover, are different for COPD patients than cancer patients and different still for total knee replacement patients. Patients experience less stress, higher quality of life, better quality of care and supportive, high-value communication. Health systems add seamless and more comprehensive patient interaction, filling gaps in communication, understanding, scheduling, outreach, education and knowledge exchange than would otherwise typically happen for patients and providers. The continuous guided dialogue from the our solution produces substantially lower costs, improved satisfaction, lower stress, higher quality, and a superior total experience for the patients and their families, as well as the associated care teams.
