Participate in clinical decision-making on cancer treatment and survivorship care plans. Services include therapeutic selection and optimization, symptom management, medication reconciliation, drug-drug interaction, adverse drug reaction management, drug information support, and other miscellaneous medication-related supports. • Provide direct patient counseling and education on antineoplastic treatments detailing the medications and their complications, potential drug interactions, adverse effects, and effective symptom management. • Work collaboratively with other health care professionals to develop institutional guidelines and standard operating procedures (SOPs), and make evidence-based decisions designed to improve patient care. • Participate on committees to improve the safety, efficacy, and quality of cancer care by developing policies and implementing programs to ensure the safety of staff and patients during the receipt, preparation, administration, and monitoring of antineoplastic agents. • Assist in precepting a group of pharmacy students in a year-long transition of care quality improvement project.
