Play a critical role as the first clinical point of contact for patients and families entering hospice care. Responsible for determining patient eligibility, conducting assessments, overseeing the entire intake and admissions process, and completing paperwork and electronic charting according to Medicare guidelines. Conduct a thorough physical assessment of the patient and review medical history, diagnoses, current medications, and functional status. Determine eligibility for hospice services under Medicare/insurance guidelines. Collaborate with physician and interdisciplinary hospice team to create the initial plan of care. Explain the philosophy and scope of hospice care to patients and families. Provide emotional support and address questions or fears about end-of-life care. Complete admission documentation and electronic charting within strict regulatory timelines. Ensure eligibility criteria are clearly documented for compliance with Medicare/Medicaid and accreditation bodies. Order necessary durable medical equipment (DME), medications, and supplies. Communicate with physicians, case managers, and hospice team members to ensure smooth transition into care. Serve as the liaison between hospital/nursing facility staff and hospice team when patient transfers from another setting.
