Choice Matters is a Medicare certified home health care agency with focus on convalescence and aging in place. We go to great lengths to understand the needs of our patients and their families. Home care is a welcome alternative to long hospital confinement or nursing homes, as more and more studies prove that patients do better and for longer periods of time, when allowed to stay in familiar surroundings.We are locally owned and operated providing services to the following counties: Duval, Clay, Baker, Nassau and St. Johns.
