   Provide compassionate care, supportive presence, active listening, spiritual and emotional support to hospice patients and their loved ones through difficult transitions in their lives, both civilian and veterans.• Make visits to be present for grieving families at the time of the death of their loved ones.• Co-facilitated bereavement support groups in Chesapeake, VA.• Co-facilitated Camp Lighthouse Bereavement Adult bereavement groups at Sentara\'s weekend day camp during the first weekend in October 2014 and 2016.• Connect with various ethnic and faith communities in Hampton Roads toward working together for End of Life issues. • Provide spiritual and pastoral care through weekly participation as a member of the interdisciplinary team for patients in Hospice.• Visits include patients or their families’ homes, facilities both Sentara and non-Sentara. These facilities are Skilled Nursing Facilities, Assisted Living Facilities, and Hospitals.• I have been available for bereavement counseling in coordination with our bereavement coordinator for our families as needed. • Complete 5-day pastoral care admissions and assessments according to Medicaid guidelines.• Develop warm supportive pastoral relationships with my patients and their families.Make initial bereavement contacts, 3, 9 and 13 month calls to bereaved clients.• Share in on call rotation, every third week after hours, with team of chaplains.
