Performing clinical activities including, pharmacokinetic dosing/monitoring, anticoagulation dosing/monitoring, IV to PO, and renal monitoring. Work with the healthcare team to optimize pharmacotherapy opportunities, formulary adherence, and reduction in poly pharmacy and associated drug cost. Processing of investigational drug studies, TPNs, and chemotherapy. Perform advanced prep review to ensure patients are receiving the appropriate antibiotic at the appropriate dose prior to surgical procedures. Also utilizing advanced prep review to ensure surgeons have the required special drugs for any surgical cases. Supervise, check and controls quality and accuracy of work performed by technicians and support staff both in central pharmacy as well as the IV room. Help to serve as Primary or Secondary preceptor to residents and pharmacy students. Also experience filling in at the smaller NH facilities as needed.
