    Our mission is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life-changing illness. Transitions LifeCare provides physical, emotional, and spiritual care to those living with an advanced illness, their caregivers, and those who have lost a loved one.Primary responsibilities include working with the CEO, the Board of Directors, the Board of Trustees, and their committees to implement comprehensive and effective fundraising strategies to achieve annual and long-term philanthropic goals for the Indigent Care program, GriefCare, and TransitionsKids. These goals include unrestricted funding through the Annual Campaign, and other restricted funds to achieve the annual budget goal of $3-6 million per year through Leadership Major Gifts, annual appeals and newsletters, third party special events, foundation grants, e-mail campaigns, and a grateful family campaign. Build a pipeline of qualified new donors through prospect research of grateful families and other constituents. Communicate effectively and maintain positive relationships with important community, business or professional individuals or groups with the Transitions LifeCare (TL) service area. Interact with the philanthropic community, private foundations, and corporations to seek new funding opportunities. Build the Endowment Fund through a comprehensive planned giving program, and Legacy Society. Supervise philanthropy department staff of 6 FT professionals.
