We also regularly collaborate with our members’ physicians’ and care teams, so we can build innovative programs and enhance existing systems that get our members the care they need, when they need it most. C3 is the only national non-profit Accountable Care Organization founded and governed by its Federally Qualified Health Center partners. With a network of 1,500 primary care physicians, and 3,200 clinicians working in Massachusetts, D.C., California, Connecticut, Louisiana, North Carolina, Oregon, and Washington state, we are intensely focused on providing dependable, convenient, and quality care in the communities where our members have chosen to live, work, and raise their families.
