Perform outreach to enrollees identified for Behavioral Health Community Partners program • Complete intake process, including Consent to Participate and Releases of information, with enrollee after successful outreach• Work with enrollee to identify individual care goals for Person Centered Treatment Plan(PCTP) to help support independence and harm reduction• Complete insurance required Comprehensive Health Assessment questionnaire with enrollee • Perform monthly (minimum) contacts with enrollees tracking care goals and tasks within electronic computer system• Connect enrollee with community resources (transportation, housing, education, employment, health etc) in support of Person Centered Treatment Plan and help enrollee navigate successfully through those resources• Communicate with enrollees PCP, Therapist, Psychiatrist, ACO/MCO and other collaterals for upcoming appointments, additional contacts points, provider input with PCTP etc
