As a Clinical RN in the Pediatric Pulmonary Clinic at Children\'s Hospital Colorado, I manage our pulmonary patient\'s care once discharged from the hospital. Responsibilities include:• Facilitating clinic visits by performing patient intake, assessment, and communicating patient needs/status to provider. Clinic visits include assessment, treatment plan and medication management, procedures, and education •Operating Nurse Triage line to facilitate urgent needs including medication needs, acute illness, emergency response, and care coordination •Fulfilling Case Management role by managing patient\'s home needs including medications, medical equipment, legal forms/documents, managing medical care for school, travel, etc. All Roles include: •Ensure a high level of customer service and response times •Build trust, partnership, and rapport with patients, families, and the interprofessional team •Use our Electronic Medical Record application Epic to correspond with families, our medical team, and any outside contacts •Document all actions and communication appropriately •Work within my scope of practice following the policies and procedures put in place by CHCO
