Board Certified Chaplain (BCC) Certified Christian Chaplains & National Association of Veterans Affairs Chaplains (NAVAC)- Retired US Air Force Chaplain; 20+ years of Chaplaincy, Leadership, and Management experience- Participates in the interview and selection process of hiring team members- Shadowed by new team members for familiarization and orientation with Hospice Philosophy- Completes all Initial Spiritual and Bereavement Risk Assessments well within CMS 5-day requirement- Calm in crises to bring peace in the storm for Patients, families/loved ones, and team members- Participating and contributing member during Interdisciplinary Team/Group (IDT/IDG) meetings - We Honor Veterans (WHV); planned Veteran Pinning/Coining Ceremonies at multiple facilities- Trained and proficient in all chaplain-related competencies; effectively collaborates with MSW, Bereavement Coordinator and Volunteer Coordinator to ensure best outcomes for our Unit of Care (Patients and families/loved ones)- Presents didactics to numerous Skilled Nursing Facilities/Rehab Centers on Hospice Philosophy, Grief/Loss, Grief Processes, and other Spiritual/Emotional/Relational matters - Provides prayer, pastoral presence, companionship, spiritual & emotional support to Patients, Family Members, Caregivers, and Staff Members during Life-Limiting and/or End-of-Life situations- Participate in Care Conferences involving Patients, Family Members, and staff members to ensure optimal care and best possible outcome for all involved- Develop Plan of Care (POC), then schedule and complete Patient Visits IAW Patient’s POC- Arrange for Volunteer Support availability for 11th Hour Companion Visits, Companionship Visits in general, or Journaling a Patient’s Life Story as requested by the Patient- Thoroughly and timely chart Patient visits, Bereavement Risk Assessments, and Condolence calls/visits same day using Epic (EMR)- Plan and officiate Celebration of Life/Memorial Services as requested
