Supporting individuals on rehabilitation/ development pathways in a “step-down” mental health care setting to develop and maintain essential daily living skills to enable them to go in to lead more independent lives in the community. Taking a person-centred approach to care planning and delivery of care. Responsible for facilitating monthly multi-disciplinary team meetings and quarterly rehabilitation pathway reviews for up to 15 service users – liaising with their circle of care (GPs, specialists, social workers, family and friends etc) to manage and support their health and medical wellbeing. Encouraging service users to participate with daily living/ wellbeing activities to aid their development (e.g. supporting/observing/ documenting medication administration, encouraging good personal hygiene/ self care, daily life skills, encouraging a balanced diet). Responsible for updating care plans, inputting information onto our system and communicating well with service users, Seniors, Care Manager and external agencies as required
