ABOUT CARRIE GRAHAM
I\'m a gerontologist and health policy researcher who has worked for over 20 years in a career focusing on improving health care, services and supports for older adults and people with disabilities. I\'m passionate about stakeholder-engaged policymaking and translating evidence and consumer voice into policy solutions. I am currently the Director of Long Term Services and Supports at the Center for Health Care Strategies, working with stakeholders and state agencies to develop and implement transformative change in services and care for older adults and people with disabilities. I am also an Adjunct Professor at the UCSF Institute for Health and Aging. My areas of expertise include: Medicare/Medicaid policy; Post-acute/LTC care quality and financing; Integrated care for dually eligible seniors and people with disabilities; Home and community-based services; Innovative models for aging in community; Community-based participatory evaluation; and Stakeholder engagement.
