   As a case manager for Dungarvin, my main duty is to help people find and obtain support and services that allow them to live independently in the community. This is done using a person-centered approach meant to support the person in making their own choices and decisions regarding the services they may need. This role requires me to act as a resource, educator, and service coordinator, but the individual and/or guardian will ultimately make any life decisions. In this role, my main focus is on helping vulnerable individuals navigate the complex service delivery system and coordinating services for them. This is done through a systematic process of assessments, planning, referral, service coordination, monitoring, consultation, and advocacy assistance in order to address the multiple service needs of each individual.Finally, this role keeps me in contact with a vast range of organizations, community members, and families on a regular basis. This is why it is also important to act in a professional manner at all times and respect and understand that I represent the interests of the individuals I support.
