Home Health & PACE Program SupportIn collaboration with the Sutter Home Health PACE Program, I support the development of compassionate, skilled caregivers within an interdisciplinary team model. My role focuses on: • Mentoring Home Health Aides to deliver high-quality, patient-centered care. • Partnering with nurses, social workers, therapists, and providers to ensure coordinated care plans. • Advocating for participants’ emotional wellness, dignity, and independence while aging in place. • Enhancing team communication to reduce burnout and improve retention. • Providing education on leadership, resilience, and professional growth for frontline caregivers.By empowering home health aides and strengthening team collaboration, I help create a culture of care where seniors thrive safely at home, and healthcare professionals feel seen, supported, and valued.
