   Conducting an average of 15 comprehensive clinical diagnostic evaluations a week of children, adult and geriatric patients in psychiatric crisis presenting in person or virtually to the Emergency Mental Health department of a high-volume, level one emergency room- Utilizing assessments, patient input, collateral information obtained from patients’ care teams, families and community supports and input from the hospital care team to determine the appropriate levels of care for patient safety and treatment- Facilitating referrals to identified levels of care, including inpatient, partial hospitalization, intensive outpatient, outpatient and specialized intervention programs- Coordinating access to and delivery of additional social services, including financial assistance, housing and health insurance, to promote sustained patient wellbeing, independence and stability- Completing robust discharge planning with patients and their supports to promote continuity of care in the community and familiarity with safety/contingency plans in the event of ongoing or increased symptomatology.
