   Created and developed North Carolina’s first hospital-based “hot spotter” model designed to engage high-risk, high-cost uninsured patients to improve health outcomes through connection to community resources, medical homes, and insurance coverage when appropriate. Utilized over $3 million in grant funding to successfully expanded models to specifically address both homeless and behavioral health patients. Over $14 million reduction in cost with over 1400 patients engaged to date • Developed predictive analytic risk models based on social and medical factors to identify high-risk patients in EPIC Healthy Planet; subsequently shared with other hospital systems. • Led teams of community case managers and peer support specialists developing innovative approaches to identify, engage and impact vulnerable patients in the community • Successfully applied to become the first hospital system to receive and implement Federal PATH program designed to engage homeless patients with severe and persistent mental illness; converted to hospital-based team to increase scope and impact in 2019 • Developed both medical and mental health medical homeless respite programs to improve outcomes and lower costs around transitions of care. • Developed grant-funded telehealth initiative to drop ER utilization by 80% from a local shelter and subsequently spread to seven other homeless programs • Established regular data review with community safety net providers to coordinate the care of patients at high risk for poor outcomes • Developed regular community-based lecture series to empower vulnerable populations through improved health education • Developed collaboration across multiple social sectors to improve coordination and address social determinants of health.
