   Provide education to each constituent on the role and benefits of the Lung Cancer Screening program and high-quality cancer care. Establish and maintain positive working relationships with key internal and external customers. Ensure appropriate and timely communication of patient progress to referring physicians and other care providers to ensure service continuity. Support providers and assist patients in understanding their treatment options and resources available. Provide behavioral and psychosocial education that are critical components of lung cancer risk reduction. Provide emotional support through the continuum of cancer care and strengthen patient-provider relationships.Participation in weekly Multi-Disciplinary Tumor Conference to evaluate lung screening results. Coordination of multiple medical services, as indicated, for follow-up care.Address logistic and psychosocial barriers that may interfere with successful adherence to the lung cancer screening protocol and smoking cessation. Empower patients to overcome barriers through a variety of modalities.Engage resources in patient scheduling, clinical resource management, patient access and others as necessary to ensure appropriate patient access and follow-up.Complete continuing education to ensure understanding of current evidence and guidelines associated with lung cancer screening and incidental nodule follow-up.
