Direct clinical operations for a home health agency, ensuring delivery of safe, high-quality care in accordance with state and federal regulations, including CMS and DPH guidelines. • Partner with executive leadership to align clinical services with strategic goals, including staffing, budget management, quality improvement, and service expansion. • Lead, mentor, and support a multidisciplinary team of nurses, physical therapists, occupational therapists and speech therapists fostering a culture of accountability, collaboration, and patient-centered care. • Oversee patient care planning, case management, and coordination to ensure timely admissions, comprehensive assessments (OASIS), and evidence-based care delivery. • Monitor compliance with documentation standards, infection control practices, and agency policies to maintain regulatory readiness and uphold best practices. • Lead quality assurance and performance improvement (QAPI) initiatives aimed at enhancing patient outcomes, reducing rehospitalizations, and improving patient and caregiver satisfaction. • Collaborate with physicians, discharge planners, and referral partners to ensure seamless transitions of care and appropriate utilization of services. • Manage clinical staffing levels, schedules, and productivity to support growth while maintaining high standards of care and staff satisfaction. • Participate in strategic planning, EMR optimization, and operational decision-making to support the agency’s mission and expansion.
