Works with medical group, health plan, and DHCS regulatory stakeholders to bring improved population level care to Kaiser Permanente’s 320K Northern California Medi-Cal patients. Top level initiatives must balance the contractual commitments of the health plan, regulatory requirements of DHCS, and operational realities of medical group while accounting for the health and well being of our members.Major accomplishments include:1. Piloting the efficacy and return of a community-based patient navigator program2. Integration of sub regional case management services with local emergency department interdisciplinary plans3. Implementation of a successful Health Homes program, and 4. Design, testing, and validation of a predictive risk stratification model that was developed with machine learning and is now being used in Kaiser Permanente Northern California, Southern California, and Hawaii. Upcoming/Current projects include:1. Designing, developing and validating a pediatric predictive risk model using a machine learning strategy, 2. Implementing and quantifying the impact of a ‘virtual patient concierge’ across different care contexts, and3. Piloting and quantifying the results of a ED psychiatric observation status pilot.
