Provide comfort, direction and support from the initial death notification through the completion of funeral service and after care if needed.Grief Education and facilitating monthly grief support group meetings, developing bereavement plans of care for families of deceased patients. Maintaining patient confidentiality according to HIPAA regulations. Coordinated with health care and prayer team to assist families with personal adjustments. Implemented activities, which ensured and enhanced the Catholic Mission. Responded to spiritual needs of patient and staff. Facilitated ethical discussions; completed medical chart documentation as required.
