Collaborating with 9 Family Health Teams in Northeastern Ontario to drive quality improvement initiatives. I manage and standardize data through the use of digital tools like Telus PS Suite EMR and Ocean, among others.Responsibilities- Work with nine Family Health Teams (FHTs) to design and implement quality improvement initiatives- Analyze healthcare data to provide actionable insights and to support evidence-based decision-making- Develop and facilitate clinic training programs to build digital tool and data tracking skillsets- Evaluate the impact of quality improvement projects and clinical programs, ensuring alignment with best practices, organizational goals and Ontario Health priorities- Successfully implement various digital tools across clinics: PrescribeIT, Ocean, AutoScribe, TELUS EMR, etc.Key Achievements- Created a novel communication pathway between 9 FHTs to enable knowledge and resource sharing- Led data standardization and cleanup projects to enhance data reporting and analysis- Designed and launched a patient navigation pilot project at the City of Lakes FHT to support patients in need of community resources and emotional support
