Stationed in the Intensive Medical Rehabilitation Unit, I support complex patients including those with ventilators and tracheostomies. I collaborate with a multidisciplinary team of nurses, physicians, PTs, OTs, and SLPs to coordinate care and help patients transition back to daily life after serious injury- Performed bi-hourly patient rounding to assess needs, assist with bathroom care, and collect vital signs - Educate families on safe patient transfers, new equipment needed for the patient, and how to assist their loved one at home- Empower patients to participate in their own care and develop independence in preparation for discharge.
