I am the clinic RN helping in our mission of Rebuilding Healthcare as it Should be. I work with a fantastic team of providers, pharmacy experts, medical assistants, scribes, phlebotomists, community health workers, medical social workers, behavioral health experts, and population health specialists, seeing to the healthcare needs of our medically complex Medicare patients. We work to prevent avoidable admissions, engage patients in Wellness Review Visits, preventative care, screening and diagnosis, chronic condition management, specialty care and navigation, acute escalations of care, and transitional care. I provide patient and caregiver education. We provide superb care and this is a great place to work!
