The Recovery Care Coordinator (RCC) is responsible for oversight for the Recovering Service Member (RSM) and family through the continuum of care. Responsibilities encompass the full spectrum of care, services, and benefits needed for holistic recovery of the whole person and life‐long continuity of care- Serves as focal point to assist the RSM in his/her course through the entire spectrum of non-medical care, management, reintegration and rehabilitation services available from the Federal Government, including services provided by the Department of Defense, the Department of Veterans Affairs, the Department of Labor, and the Social Security Administration- Communicates with the RSM and his/her family or other individuals designated by the RSM regarding non-medical matters that arise during his/her recovery, rehabilitation, reintegration and/or transition- In accordance with current DoD and VA Interagency Care Coordination, serve as the lead coordinator responsible for the development and delivery of services/resources through the Comprehensive Recovery Plan (CRP) to ensure quality care and accountability as soon as the RSM transitions to outpatient status- Conducts a needs assessment for the RSM and family to identify goals and ensure needs are met. Based on RSM’s needs, develop, implement, and provide oversight of the CRP as defined in regulatory guidance.
