Forwards documents to registration area- Obtains and scans information provided by EMS into the patients’ EPIC record- Interacts with physicians and Emergency Room personnel in performance of referral duties- Coordinates follow up care of patients as they are discharged from the E.D. in the referral work queues. Makes and assigns follow-up referrals in the required clinic- Checks patients’ insurance to verify eligibility and benefits to determine if there are any Managed Care issues- Identifies PCP, schedules primary care services according to PCP verification process, and escalates and PCP issues to management- Coordinates outside PCP referrals- Assists with billing projects, authorizations, and clerical tasks- Maintains strict confidentiality.
